10-15Mountain View Hospice is seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join our compassionate team.
As an RN Case Manager, you will play a vital role in planning, organizing, and directing hospice care while emphasizing community health education and experience.
Our mission is to provide quality care to individuals and families within their homes and communities, ensuring comfort and support during their journey.
Key
Responsibilities:
- Plan and organize hospice care for patients and their families.
- Utilize community resources to design and implement services tailored to individual needs.
- Provide nursing care and support, maintaining a high standard of professionalism and empathy.
Requirements:
- Current licensure to practice professional nursing in the state.
- Graduate of a National League for Nursing accredited school of nursing.
- Current CPR certification.
- Valid driver's license with an insured automobile in good working order.
- Minimum of two years of nursing experience, with at least one year in public health, home care, or hospice nursing preferred.
